An Indiana medical power of attorney, also known as “Health Care Representative Appointment,” grants power to one person (a “health care representative”) to make medical decisions on another person’s (a “principal”) behalf if the latter is incapable of doing so for themselves. In addition to delivering this authority, this form allows the principal to offer documentation of their health care instructions and preferences as well as provisions for end-of-life preferences. Generally, putting such directives in writing will remove most ambiguities regarding healthcare choices, assuming a complete report of these wishes is supplied.
