An Ohio 30-Day Notice to Quit (Health or Safety Violation) is a form used when a tenant materially affects the health and safety within the premises to cause harm to themselves or other tenants. The notice must specify the acts and omissions that implicate the noncompliance and that the rental agreement will terminate not less than 30 days from delivery of notice if the tenant fails to remedy the violation(s).
