Step 1 – Date and Parties –

  • Enter the date that the document is created in dd/mm/yyyy format
  • Enter the county in which the vehicle is located
  • Enter the State in which the county and vehicle are located
  • Check the appropriate box that applies to method of payment
  • Payment/Trade-in/Gift

Step 2 – Vehicle Description –

  • Make
  • Mode;
  • Body Type
  • Year
  • Color
  • Odometer Reading (in miles)

Step 3 – Vehicle Identification Number (VIN)

  • Enter the 17 digit Vehicle Identification Number
  • Read the remainder of the section and check the applicable box (taxes included or taxes not included)
  • Other Terms and Conditions – Enter any other terms and condition not recorded on any other document

Step 4 – Odometer Disclosure Statement –

  • Enter the name(s) of the Seller(s)
  • Enter the accurate Vehicle Identification Number
  • If the reading is not able to be properly recorded check the appropriate box
  • Read the “as is” statement

Step 5 – Signatures –

  • Enter the Buyer(s) Signatures
  • Printed Name(s)
  • AND
  • Enter the Seller(s) Signature(s)
  • Printed Name(s) of the Seller(s)
  • AND
  • Witnesses 1 and 2, must enter their signature(s)
  • Provide the printed names of each Witness

Step 6 – Certificate of Acknowledgement – (Notary)

Once the document and the sale have been completed, the Notary would record the identification of all signing parties the license Notary Public will witness and acknowledge all signatures by then completing all of the required notary information to include the commission information and by affixing the state seal.
